Early Genioplasty, Guiding Growth Redefining Balance

Early genioplasty is emerging as a relevant therapeutic option in growing patients presenting with hyperdivergent patterns and orofacial functional imbalances. In children and adolescents, the timing of the intervention is critical: when performed during an active growth phase, a subtle reorientation of the chin can influence mandibular growth direction and neuromuscular balance. The identification of this optimal window relies on a comprehensive assessment combining 3D facial analysis, skeletal maturity evaluation, and functional examination. Rather than a purely aesthetic procedure, orientation genioplasty acts as a growth-guiding tool, contributing to the restoration of muscular, labial, and respiratory balance, while improving facial harmony and profile perception. Through illustrative clinical cases, this presentation highlights how a well-timed, minimally invasive intervention can achieve stable morphofunctional benefits and, in selected cases, reduce the need for more invasive corrective procedures later in life.
